Toutefois&#x002C; certains vaccins sont recommand\u00e9s. Cet article d\u00e9crit les patients qui rel\u00e8vent de cette cat\u00e9gorie&#x002C; les vaccins concern\u00e9s ainsi que les principales recommandations.<\/strong><\/span><\/p>\n<div class='summary'>\n<div class='summary-article-content'>\n<p><span style='color:black;'><span style='font-family:;' lang='FR'><strong>Messages cl\u00e9s<\/strong><\/span><\/span><\/p>\n<ul>\n<li><span style='color:black;'><span style='font-family:;' lang='FR'>Les vaccins vivants att\u00e9nu\u00e9s sont <strong>contre-indiqu\u00e9s<\/strong> chez les patients immunod\u00e9ficients&#x002C; comme lors d\u2019un traitement immunosuppresseur.<\/span><\/span><\/li>\n<li><span style='color:black;'><span style='font-family:;' lang='FR'>Les vaccins vivants att\u00e9nu\u00e9s sont les vaccins contre la varicelle&#x002C; le rotavirus&#x002C; la fi\u00e8vre jaune&#x002C; la dengue&#x002C; le chikungunya&#x002C; la tuberculose (BCG) et le vaccin combin\u00e9 contre la rougeole&#x002C; la rub\u00e9ole et les oreillons.<\/span><\/span><\/li>\n<li><span style='color:black;'><span style='font-family:;' lang='FR'>Chez certains patients pour lesquels un traitement<strong> immunosuppresseur est pr\u00e9vu<\/strong>&#x002C; une vaccination <strong>pr\u00e9alable<\/strong> avec un vaccin vivant att\u00e9nu\u00e9 est fortement recommand\u00e9e. Le cas \u00e9ch\u00e9ant&#x002C; le vaccin doit \u00eatre administr\u00e9 <strong>au moins 4&nbsp;semaines avant le d\u00e9but<\/strong> de l\u2019immunosuppression.<\/span><\/span><\/li>\n<li><span style='font-family:;' lang='FR'><strong>Apr\u00e8s l\u2019arr\u00eat<\/strong> des immunosuppresseurs&#x002C; il faut attendre plusieurs semaines <\/span><span style='color:black;'><span style='font-family:;' lang='FR'>&#x002C; voire plusieurs mois&#x002C; avant de pouvoir administrer des vaccins vivants att\u00e9nu\u00e9s.<\/span><\/span><\/li>\n<li><span style='color:black;'><span style='font-family:;' lang='FR'>Toutefois&#x002C; les vaccins vivants att\u00e9nu\u00e9s peuvent \u00eatre utilis\u00e9s chez des patients atteints de <strong>maladies (auto-)immunes chroniques&nbsp;<\/strong>non trait\u00e9s par immunosuppresseurs (\u00e0 l\u2019exception du lupus actif).<\/span><\/span><\/li>\n<li><span style='color:black;'><span style='font-family:;' lang='FR'>L\u2019<\/span><\/span><a href='https:\/\/www.hgr-css.be\/fr\/avis\/9158\/patients-id-et-vaccination'><span style='color:black;'><span style='font-family:;' lang='FR'>Avis 9158<\/span><\/span><\/a><span style='color:black;'><span style='font-family:;' lang='FR'> du Conseil sup\u00e9rieur de la sant\u00e9 (2019) \u00e9met des recommandations sur la vaccination des patients immunod\u00e9ficients.<\/span><\/span><\/li>\n<li><span style='color:black;'><span style='font-family:;' lang='FR'>En cas de doute sur l\u2019administration ou non de vaccins vivants&#x002C; il est recommand\u00e9 de demander l\u2019avis d\u2019un sp\u00e9cialiste.<\/span><\/span><\/li>\n<\/ul>\n<\/div>\n<\/div>\n<h2><span style='font-family:;' lang='FR'>\u00c9tude de cas&nbsp;: le vaccin contre la rougeole chez un patient greff\u00e9<\/span><\/h2>\n<p><span style='font-family:;' lang='FR'>Le CBIP a r\u00e9cemment re\u00e7u le message d\u2019un n\u00e9phrologue qui avait re\u00e7u une question d\u2019un patient greff\u00e9 concernant le vaccin contre la rougeole. En raison de l\u2019augmentation actuelle de l\u2019incidence de la rougeole&#x002C; son m\u00e9decin g\u00e9n\u00e9raliste a recommand\u00e9 \u00e0 ce patient de se faire vacciner contre cette maladie. Toutefois&#x002C; le patient avait subi une transplantation de rein et \u00e9tait sous traitement immunosuppresseur. Il a donc contact\u00e9 son n\u00e9phrologue&#x002C; qui lui a d\u00e9conseill\u00e9 de se faire vacciner. En effet&#x002C; le vaccin contre la rougeole est un vaccin vivant att\u00e9nu\u00e9 et donc contre-indiqu\u00e9 chez ce patient.<\/span><br \/>&nbsp;<\/p>\n<h2><span style='font-family:;' lang='FR'>Quels sont les vaccins vivants att\u00e9nu\u00e9s&nbsp;?<\/span><\/h2>\n<p><span style='font-family:;' lang='FR'>Les vaccins suivants sont des vaccins contenant des agents pathog\u00e8nes vivants att\u00e9nu\u00e9s&nbsp;:<\/span><span style='font-family:;' lang='NL'>&nbsp;<\/span><\/p>\n<ul>\n<li><span class='List1Char' style='font-size:12.0pt;line-height:150%;' lang='FR'>Vaccins antiviraux contre <strong>la varicelle<\/strong>&#x002C; contre le <strong>rotavirus<\/strong>&#x002C; contre la <strong>fi\u00e8vre jaune<\/strong>&#x002C; contre la <strong>dengue<\/strong>&#x002C; contre le<\/span><span style='font-family:;' lang='FR'> <strong>chikungunya<\/strong><\/span><\/li>\n<li><span style='font-family:;' lang='FR'>Vaccin antibact\u00e9rien contre la <strong>tuberculose (BCG<\/strong>)<\/span><\/li>\n<li><span style='font-family:;' lang='FR'>Vaccins combin\u00e9s contre <strong>la rougeole&#x002C; la rub\u00e9ole et les oreillons&nbsp;<\/strong>et contre la <strong>rougeole&#x002C; la rub\u00e9ole&#x002C; les oreillons et la varicelle<\/strong><\/span><\/li>\n<\/ul>\n<p><span style='font-family:;' lang='FR'>La composition des vaccins peut \u00eatre trouv\u00e9e dans le r\u00e9pertoire&#x002C; au niveau des sp\u00e9cialit\u00e9s.<\/span><\/p>\n<figure class='image'><img style='aspect-ratio:677\/144;' src='https:\/\/www.bcfi.be\/wp-content\/uploads\/2026\/01\/vivant_attenues.png' width='677' height='144'><\/figure>\n<h2><span style='font-family:;' lang='FR'>Contre-indications des vaccins vivants att\u00e9nu\u00e9s<\/span><\/h2>\n<p><span style='font-family:;' lang='FR'>Les vaccins vivants att\u00e9nu\u00e9s sont <strong>contre-indiqu\u00e9s<\/strong> dans les cas suivants&nbsp;:<\/span><\/p>\n<ul>\n<li><span style='font-family:;' lang='FR'>Chez les patients atteints d\u2019<strong>immunod\u00e9ficience<\/strong>&nbsp;: notamment hypo- ou agammaglobulin\u00e9mie&#x002C; leuc\u00e9mie ou lymphome&#x002C; ou une infection <strong>non contr\u00f4l\u00e9e<\/strong> par le VIH (CD4 &lt; 15&nbsp;%).<\/span><\/li>\n<li><span style='font-family:;' lang='FR'>Pendant un traitement par <strong>immunosuppresseurs<\/strong>&#x002C; corticost\u00e9ro\u00efdes \u00e0 doses \u00e9lev\u00e9es (\u226510 mg de prednisone ou \u00e9quivalent) pendant \u226514&nbsp;jours&#x002C; des antitumoraux ou radioth\u00e9rapie \u00e9tendue.<\/span><\/li>\n<\/ul>\n<p><span style='font-family:;' lang='FR'>Dans les cas ci-dessus&#x002C; il existe un risque de r\u00e9plication du virus vaccinal&#x002C; avec un risque d\u2019infection invasive et de complications graves.<\/span><\/p>\n<h3><span style='font-family:;' lang='FR'>Qu\u2019entend-on par \u00ab&nbsp;immunosuppresseurs&nbsp;\u00bb&nbsp;?<\/span><\/h3>\n<p><span style='font-family:;' lang='FR'>Liste non exhaustive d\u2019<strong>immunosuppresseurs<\/strong> pouvant \u00eatre prescrits et d\u00e9livr\u00e9s en ambulatoire (par voie orale ou sous-cutan\u00e9e)&#x002C; bas\u00e9e sur l\u2019<\/span><a href='https:\/\/www.hgr-css.be\/nl\/advies\/9158\/ic-patienten-en-vaccinatie'><span style='font-family:;' lang='FR'><u>avis 9158<\/u><\/span><span style='color:windowtext;'><span style='font-family:;' lang='FR'> du CSS&nbsp;:<\/span><\/span><\/a><\/p>\n<ul>\n<li><span lang='FR'>M\u00e9thotrexate<\/span><\/li>\n<li><span lang='FR'>L\u00e9flunomide<\/span><\/li>\n<li><span style='font-family:;' lang='FR'><strong>Immunosuppresseurs en cas de transplantation&nbsp;:&nbsp;<\/strong>azathioprine&#x002C; apr\u00e9milast&#x002C; v\u00e9dolizumab<\/span><br \/>&nbsp;<\/li>\n<\/ul>\n<h2><span style='font-family:;' lang='FR'>Les patients atteints de maladies auto-immunes peuvent-ils \u00eatre vaccin\u00e9s&nbsp;?<\/span><\/h2>\n<ul>\n<li><span style='font-family:;' lang='FR'>Les <strong>maladies (auto-)immunes chroniques<\/strong> ne sont g\u00e9n\u00e9ralement pas (ou peu) immunosuppressives. Les patients atteints de maladies (auto-)immunes inflammatoires chroniques qui <strong>ne sont pas<\/strong> <strong>trait\u00e9s<\/strong> par des immunosuppresseurs peuvent donc \u00eatre <strong>vaccin\u00e9s<\/strong> \u00e0 l\u2019aide d\u2019un vaccin vivant att\u00e9nu\u00e9. Le vaccin n\u2019affectera pas la progression de la maladie (r\u00e9f.&nbsp;: <\/span><a href='https:\/\/www.hgr-css.be\/fr\/avis\/9158\/patients-id-et-vaccination'><span style='font-family:;' lang='FR'>Avis&nbsp;9158 du CSS&#x002C; 2019).<\/span><\/a><span style='font-family:;' lang='FR'>&nbsp; Les m\u00eames vaccins que pour la population g\u00e9n\u00e9rale sont recommand\u00e9s chez ces patients.<\/span><\/li>\n<li><span style='font-family:;' lang='FR'>Le <strong>lupus actif<\/strong> constitue une exception. C\u2019est pourquoi il ne faut <strong>pas&nbsp;<\/strong>administrer de <strong>vaccins vivants<\/strong> chez des patients atteints de lupus.<\/span><\/li>\n<li><span style='font-family:;' lang='FR'>En ce qui concerne le <strong>vaccin contre la fi\u00e8vre jaune<\/strong>&#x002C; la <strong>myasth\u00e9nie grave<\/strong> est \u00e9galement une <strong>contre-indication<\/strong>.<\/span><br \/>&nbsp;<\/li>\n<\/ul>\n<h2><span style='font-family:;' lang='FR'>Quand faut-il retarder une vaccination avec des vaccins vivants&nbsp;?<\/span><\/h2>\n<ul>\n<li><span style='font-family:;' lang='FR'><strong>Apr\u00e8s l\u2019arr\u00eat de<\/strong> <strong>m\u00e9dicaments immunosuppresseurs<\/strong>&#x002C; des vaccins vivants att\u00e9nu\u00e9s ne peuvent \u00eatre administr\u00e9s imm\u00e9diatement. Il y a lieu d\u2019attendre plusieurs semaines&#x002C; voire plusieurs mois&#x002C; en fonction de la demi-vie du m\u00e9dicament et de la dur\u00e9e de l\u2019effet immunosuppresseur. Un aper\u00e7u non exhaustif des intervalles entre l\u2019arr\u00eat des immunosuppresseurs et l\u2019administration en toute s\u00e9curit\u00e9 de vaccins vivants att\u00e9nu\u00e9s est pr\u00e9sent\u00e9 aux p 46-48 de l\u2019<\/span><a href='https:\/\/www.hgr-css.be\/fr\/avis\/9158\/patients-id-et-vaccination'><span style='font-family:;' lang='FR'>avis 9158<\/span><\/a><span style='font-family:;' lang='FR'> du CSS (2019).<\/span><\/li>\n<li><span style='font-family:;' lang='FR'>Chez les <strong>nourrissons expos\u00e9s in utero \u00e0 des agents biologiques<\/strong> (anticorps monoclonaux&nbsp;: inhibiteurs du TNF&#x002C; antagonistes des interleukines)&#x002C; la vaccination avec des vaccins vivants doit \u00eatre retard\u00e9e <strong>jusqu\u2019\u00e0 l\u2019\u00e2ge de 6&nbsp;mois<\/strong> si l\u2019administration de l\u2019agent biologique a \u00e9t\u00e9 poursuivie apr\u00e8s la 22<sup>e<\/sup> semaine de grossesse. Ces enfants ne pourront donc pas \u00eatre vaccin\u00e9s contre le rotavirus. Pour l\u2019infliximab&#x002C; il est recommand\u00e9 d\u2019attendre 12&nbsp;mois apr\u00e8s la naissance&#x002C; sauf si le taux s\u00e9rique d\u2019infliximab est ind\u00e9tectable chez le nourrisson (<\/span><a href='https:\/\/www.cbip.be\/fr\/articles\/query?number=F48N03C'><span style='font-family:;' lang='FR'>cf. Folia mars&nbsp;2021<\/span><\/a><span style='font-family:;' lang='FR'>).<\/span><br \/>&nbsp;<\/li>\n<\/ul>\n<h2><span style='font-family:;' lang='FR'>Quand vacciner les patients immunod\u00e9ficients&nbsp;?<\/span><\/h2>\n<ul>\n<li><span style='font-family:;' lang='FR'>Chez les patients pour lesquels un traitement immunosuppresseur est pr\u00e9vu&#x002C; une vaccination <strong>pr\u00e9alable<\/strong> avec un vaccin vivant att\u00e9nu\u00e9 est fortement recommand\u00e9e. Le cas \u00e9ch\u00e9ant&#x002C; le vaccin doit \u00eatre administr\u00e9 au moins 4&nbsp;semaines avant le d\u00e9but de l\u2019immunosuppression. Cela s\u2019applique aux patients qui vont subir une <strong>transplantation d\u2019organe<\/strong> (transplantation d\u2019organe solide) et aux patients atteints de maladies immunitaires inflammatoires qui seront trait\u00e9s par immunosuppresseurs. Chez ces patients&#x002C; le vaccin contre la <strong>rougeole&#x002C; la rub\u00e9ole&#x002C; les oreillons et la varicelle<\/strong> est fortement recommand\u00e9 par le CSS. Cette vaccination peut \u00e9ventuellement \u00eatre administr\u00e9e selon un calendrier acc\u00e9l\u00e9r\u00e9 (voir <\/span><a href='https:\/\/www.hgr-css.be\/fr\/avis\/9158\/patients-id-et-vaccination'><span style='font-family:;' lang='FR'>l\u2019Avis 9158<\/span><\/a><span style='font-family:;' lang='FR'> du CSS (2019)). Remarque&nbsp;: en cas de voyage futur&#x002C; il est fortement recommand\u00e9 de consulter une clinique de voyage bien \u00e0 l\u2019avance.<\/span><\/li>\n<li><span style='font-family:;' lang='FR'>Chez les patients ayant subi une <strong>transplantation de cellules souches h\u00e9matopo\u00ef\u00e9tiques<\/strong>&#x002C; la vaccination contre <strong>la rougeole&#x002C; la rub\u00e9ole&#x002C; les oreillons et la varicelle<\/strong> est fortement recommand\u00e9e par le CSS au moins 24&nbsp;mois apr\u00e8s la transplantation&#x002C; en l\u2019absence de m\u00e9dicaments immunosuppresseurs ou de maladie du greffon contre l\u2019h\u00f4te. Deux doses de vaccin sont n\u00e9cessaires \u00e0 un mois d\u2019intervalle.<\/span><\/li>\n<li><span style='font-family:;' lang='FR'>Chez les <strong>patients s\u00e9ropositifs dont le taux de CD4 est \u2265 15&nbsp;%<\/strong>&#x002C; le vaccin contre <strong>la rougeole&#x002C; les oreillons&#x002C; la rub\u00e9ole et la varicelle<\/strong> est fortement recommand\u00e9 par le CSS.
